Visiting Postgraduate Medicine Learners

Completion of the Postgraduate Medicine Health Screening Record is a requirement for elective learners. See Postgraduate Medical Students tab on the left to access the form and detailed instructions on submitting documentation.

McMaster Undergraduate Medicine Learners

The Health Screening Office completes the AFMC Student Portal Immunization and Testing form for all McMaster medical students, no appointment is necessary and there is no charge to students for this service.  

      • The completed form is automatically uploaded to MedSIS for learners who are fully cleared on their Year One Health Screening Record. Learners who are provisionally cleared at the time they apply for electives must request the form from the Health Screening Office.

      • Check over your form and report any possible errors to the Health Screening Office, then complete Section A (Student Declaration) and any attached appendices before uploading the form to your profile on the Portal.

      • Once the form is completed by the Health Screening Office, any additional requirements must be documented by another health care provider (including influenza immunization in Section I).
      • Individual schools may have additional requirements which are not part of the basic AFMC requirements, e.g. updated TST, COVID-19 vaccines/booster, updated HIV/HCV serology. These are listed on each school’s Institution Profile and the AFMC Student Portal Immunization page https://afmcstudentportal.ca/Immunization/.
