Injury/Incident Report Form
The McMaster Injury/Incident Report must be completed and submitted within 24 hours of an injury. The form must be downloaded and saved to your computer before filling. Provide the requested the information, print it and obtain the indicated signatures. During the COVID19 pandemic, please email the pdf to FHSSO. Our office is working remotely and our fax machine has been retired.

To view your MOSAIC training record, log in to Mosaic and click Regulatory Training > Training Summary.
Safety Programs
All of McMaster's safety programs are captured in the Risk Management Manual. It is a series of documents which outline our individual responsibilities related to a variety of safety topics. All staff and students should familiarize themselves with this website and the documents within. If you have questions about any of the programs or how to implement them within your department, please contact FHSSO.

Code Awareness
Code Awareness for personnel working in hospital-hosted locations is required annually and is now bundled with FHS Hospital Fire Safety.
Emergency Code Response
Posters have been created for Emergency Code Response for a laboratory and an office setting. Print and post the information appropriate for your area.
