This project represents a partnership involving McMaster University School of Nursing, Hamilton Health Sciences and Grey Bruce Health Services and Nursing Health Services Research Unit at McMaster University to develop, implement and evaluate an innovative interprofessional 12-week mentorship program involving more than ten disciplines to foster collaboration and to promote integrated patient-centred care. This inaugural program will provide a model for cluster mentoring (4 different disciplines in each cluster) that will enable health care team members to acknowledge individual strengths and differences so that the team can achieve purposeful shared patient-centred outcomes
