We are pleased that you are considering Midwifery Education at McMaster University.

Below are a list of steps that will constitute a complete application to the program.

STEP 1 - OUAC APPLICATION

Current Ontario Secondary School Students must apply using the Ontario University Application Form OUAC 101 - www.ouac.on.ca/101

If you are NOT a Current Secondary School Student, you will need to complete the Ontario Universities Application Form OUAC 105 - www.ouac.on.ca/105

STEP 2 - SUPPLEMENTARY APPLICATION

 

Once you have successfully completed your OUAC application, you will need to apply to the Midwifery Education Program Supplementary Application, using the on-line application form. You will require your OUAC Reference Number to be able to complete this form.

 

 

 

Current McMaster Students: If you are a current McMaster Student (not eligible to graduate at the end of your current year), you will not be required to apply to the Ontario Universities Application Centre. Instead, please complete the on-line Midwifery Supplementary Application Form. You will require your McMaster Student Number to complete the form. Also, no supporting documents are required.

 

McMaster Second Degree: If you are a McMaster graduate or potential graduate and wish to pursue a second undergraduate degree (providing you have not attended another university or received a college diploma since last registered at McMaster); Use the McMaster Returning Student Application to apply online at:

STEP 3 - SUPPORTING DOCUMENTS

 

Supporting documents are required for admission consideration to McMaster University. Before an application can be assessed, we require official transcripts from an applicant's high school and all post-secondary schools attended.

 

PLEASE NOTE: CURRENT HIGH SCHOOL APPLICANTS COMPLETING THEIR FINAL YEAR, DO NOT NEED TO REQUEST AN OFFICIAL TRANSCRIPT. Grades will be sent automatically to our Registrar's Office.

 

All required official transcripts must be received by McMaster Admissions before an applicant's file can be deemed complete and ready for review.

 

Transcripts must be submitted by the issuing institution directly to: McMaster University, Admissions, GH108, 1280 Main Street West, Hamilton, ON CANADA L8S 4L8.

THE ABOVE 3 STEPS NEED TO BE COMPLETED BY FEBRUARY 1, 2012
