Outpatient ID Clinics

These clinics should provide an opportunity for graded responsibility.

R4 (ID/MM) R2 (MM) – residents should seen patients on their own, review cases with attending and dictate consultations. Resident should be expected to follow-up on tests they order. The patient interactions will be more closely supervised than in future years and all work must be reviewed by the attending physician.

R3 (MM) – residents may begin to take on their own patients and should be encouraged to make management decisions. Interactions will still be supervised closely however residents may be given more responsibility for follow-up.

R5/R6 (ID/MM) R4/R5 (MM) – residents should have their own patients in the clinic and be responsible for management decisions, following discussion with the attending. Residents should be responsible for follow-up and trouble shooting any difficulties their patients encounter between clinics. Residents should respond to any calls from the outpatients and trouble shoot difficulties they encounter in the community.

Medical Expert

  1. To learn an approach to diagnosing and managing infectious diseases in the outpatient setting.
  2. To follow through, dictate, collate and synthesize data during clinics and in follow-up.

Communicator

  1. To learn how to communicate with patients and their families in the outpatient setting. Ideally this communication should take place during clinics and between clinics by following up on calls made from patients and families to the attending physician’s office.
  2. To gain experience in counseling patients around difficult issues (chronic infections, the importance of compliance with antibiotics, diabetes control, smoking and alcohol cessation).

Collaborator

  1. To communicate effectively with clinic staff, referring physicians, home care workers and laboratory staff directly and through written consultation notes.
  2. To ensure that the patient has effective follow-up and is connected with appropriate supports in their community.

Manager

  1. To learn how to effectively manage patients in a limited amount of time and with the limited resources available.
  2. To learn how to effectively run an outpatient clinic and understand the importance of documentation in this practice setting.

Scholar

  1. To apply an evidence based approach to outpatient management.
  2. To apply newly- learned information to patient care

Professional

  1. To be professional with all patients and staff at all times, including appropriate respect for confidentiality.
  2. To understand and respect diversity at all levels

Health Advocate

  1. To understand how physicians can advocate on behalf of individual patients as well as for the larger patient population
  2. To assist patients in accessing medications and address the other determinants of health which impact their care.
